Notice of Privacy Practices

SNT Biotech Lab is committed to protecting the privacy of your protected health information ("PHI"). We are required by law to maintain the privacy of your PHI, to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.

How we may use and disclose your PHI

  • Treatment: we may use and disclose PHI to provide laboratory testing services and to coordinate with the healthcare providers involved in your care.
  • Payment: we may use and disclose PHI to obtain payment for services, for example by billing your health plan.
  • Healthcare operations: we may use and disclose PHI for quality assessment, accreditation, training, and other operational activities.
  • As required or permitted by law: including for public health activities, health oversight, judicial proceedings, and to avert a serious threat to health or safety.

Your rights regarding your PHI

  • The right to request access to and a copy of your PHI.
  • The right to request an amendment of your PHI.
  • The right to request an accounting of certain disclosures.
  • The right to request restrictions on certain uses and disclosures.
  • The right to request confidential communications.
  • The right to receive a paper copy of this Notice.
  • The right to be notified in the event of a breach of unsecured PHI.

Our duties

We are required to maintain the privacy of your PHI, provide this Notice, abide by its terms, and notify you if we are unable to agree to a requested restriction. We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as any information we receive in the future.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
